We support young people aged 16–25, individuals transitioning from children’s services and families planning future support, providing personalised guidance that reflects each person’s needs, goals and aspirations.
Our approach focuses on early transition planning, active family involvement, gradual independence building and joint working with relevant services to ensure a coordinated, person-centred and seamless transition into adult care and support.
Each personalised support plan may include transition pathways, skills development, risk management and future accommodation planning to help individuals prepare for adult life with confidence and stability.
Ideally from age 14–16.
We aim to help individuals achieve a smooth transition between services, reduce disruption, increase independence and develop the confidence needed to thrive in adulthood.
