Our Emergency Care service supports individuals who require urgent assistance following a sudden hospital discharge, carer breakdown, family emergency, acute deterioration in health, or safeguarding intervention. Whatever the situation, we respond promptly to provide the appropriate level of care while working closely with families and healthcare professionals to ensure a safe and effective care plan is put in place.
Our approach focuses on delivering rapid, flexible and person-centred support when it is needed most. We aim to carry out a prompt assessment, often on the same day where possible, deploy experienced care professionals quickly, implement short-term stabilisation care plans and, where required, support a smooth transition into ongoing long-term care. Every emergency response is tailored to the individual’s immediate needs while planning for their continued wellbeing.
Our Emergency Care service includes urgent domiciliary care, emergency live-in care placements, overnight support, assistance with medication and personal care in accordance with agreed care plans, and coordination with hospital discharge teams and other healthcare professionals. We provide compassionate, reliable support that helps individuals remain safe, comfortable and well cared for during times of crisis.
Our aim is to provide safe and immediate support, reduce unnecessary delays in hospital discharge, stabilise care needs quickly and ensure a seamless transition to ongoing care where required. Through our responsive and compassionate approach, we help individuals and families navigate unexpected situations with confidence, reassurance and continuity of care.
